vpx[dec|enc]: Extract IVF support from the apps.

- Move IVF reading support into ivfdec.c and ivfdec.h
- Move IVF writing support into ivfenc.c and ivfenc.h
- Removed IVF writing code from the SVC example in favor of ivfenc.
