Differentiate 2 refresh frame parameters

refresh_frame_flags is used in both encoder and decoder to represent
refresh frame bit mask.

In encoder side, there is another parameter: cpi->refresh_frame, which
is used to store refresh frame information. In functions that use
cpi->refresh_frame, a local variable is defined as following.
const RefreshFrameFlagsInfo *const refresh_frame_flag = &cpi->refresh_frame;

To eliminate this name confusion, here we change this local variable to
be refresh_frame (i.e. use the same name as in cpi).
